Shift: 8:00 AM - 4:30 PM
Experience:
60 months of role experience is required.
60 months of Case Management experience is required.
Experience with Epic is required.
Requirements:
Candidates must have a Mississippi license or compact license in hand at the time of application.
This role may require floating to additional units and locations
Candidates must live at least 50 miles away from this facility in order to be considered a traveler when applying for this role.

Complex Case Manager Discharge Planning
Guaranteed 40 hours per week
Shift: Mon-Fri
No Weekend Requirements.
Holiday Requirements: Rotated as needed
Prefer RN BSN. Would consider ADN with extensive Complex Case Management experience.
Case load/number of patients per day/consults per day 15-20
Do you require InterQual criteria or Milliman experience: No, but need to have general understanding of discharge readiness
Discharge planning for complex patient populations, no UR piece to the role. Potential to plan discharge to a variety of setting including home, home with home health, SNF, LTAC, hospice, etc.
Will cover all Inpatient units throughout the hospital .
Experience or Must have' skills: Must be RN Case manager with 5 years. Acute Care Management experience with previous experience in large academic Medical Center hospital. UMMC is the only level 1 trauma hospital in the state so will receive a variety of complex patients and those with no other hospital options.
